    Minimum 1+ years pharmacy experience required.

    Ambulatory Care Pharmacist Job at Pennsylvania hospital.
    In this role, the pharmacist will focus on working with patients at the health system to manage medication regimens (esp. on target disease states--- i,e.. Diabetes).

    Position: Ambulatory Pharmacist

    Location: Pennsylvania

    Start-date: ASAP
    Duties:

    • Receives and interprets physicians' medication orders
    • Provides drug information to other health care professionals and patients
    • Assumes responsibility for the delivery of pharmaceutical care to patient groups served by the particular pharmacy in which the pharmacist is scheduled to work
    • Contributes to the quality and effective operation of the Pharmacy Department
    • Provide clinical support of hospital pharmacy function in a fast paced environment
    • Maintains adequate drug supply at pharmacy at all times
    • Work as part of the interdisciplinary clinical team
    • Correctly inform and instruct patients on any drugs to be used after they are discharged

    Requirements: PA licensed, 1 years + of inpatient/clinical/infusion experience, FT availability starting ASAP, good attitude and people skills

    Education: BS in Pharmacy or PharmD.
