    One of our client, a non-profit, multi service agency with a flagship site in Bronx, NY is seeking for an Program Director Respite

    Summary:


    Under the supervision of the Chief Program Officer, the Program Director will lead the overall operation of our medical Respite service programs, which includes management, planning, development, implementation, monitoring, evaluation of policies and implementing proper procedures for the program.


    The Respite Director has the direct administrative oversight for the day-to-day operations of the Respite Program which provides short-term, residential, post-medical care for homeless persons who are not ill enough to remain hospitalized, but who are too frail to be discharged to the shelter system.


    The Director supervises Case Managers and handles all Respite Program functions including assurance of case management documentation, external networking/communication with contracted entities (hospitals and Managed Care Organizations).

    The Program Director reviews and manages all referrals and also conducts screenings and intakes; ensures that Respite clients receive appropriate assessment; Support Plans and case management services to enable them to move towards the levels of independent living they are able to achieve


    Job Qualifications
    Requires broad knowledge of residential care for persons with chronic illness; both medically and behaviorally


    Requires knowledge of behavioral health practices and procedures; psychosocial rehabilitation; psycho-education; chemical dependency, motivational enhancement strategies; assessment, crisis intervention, process facilitation and supportive counseling and case management activities.

    Requires strong knowledge of the NYC housing system.


    Extensive knowledge of principles and practices of medical respite/diversion programs includes all relevant guidelines, funder regulations and directives, as applicable to the program.

    Thorough knowledge of program development, implementation methodology, and research methods and techniques.

    Ability to plan, organize and direct the work of clinical and administrative staff.

    Community minded, patient, creative, flexible, and compassionate.


    Culturally sensitive to persons who are diagnosed and afflicted with medical issues, various national/ethnic backgrounds, sexual orientation, religion, and substance abuse issues.


    Ability to deal tactfully and diplomatically with other Comunilife employees, federal, state, and city officials, professional and technical groups, clients, and the general public.

    Strong project management skills with the ability to administer multiple projects simultaneously and prioritize workload

    Works effectively as a team member with other members of department and agency leaders.

    Knowledge of NYC hospitals and other potential referrals sources.

    Ability to handle confidential information with discretion and integrity.


    Strong project development and management skills, including managing competing priorities and meeting deadlines; ability to adapt strategy to changing conditions and communicate changes effectively.

    Highly motivated, with proven capacity to work independently, take initiative, and thrive in high-paced environment.


    Experience collaborating in a multi-disciplinary, diverse team, maintaining a healthy work environment for staff that encourages professionalism, respect, equality, diversity and continuous improvement.

    Willingness to travel among program spaces, and community partner sites

    Computer proficiency in Microsoft Office, Google Docs, Power Point and AWARDS


    Education and Experience Requirement(s):
    Masters Degree in behavioral health, social work, public health, health administration, or a related field.


    Five years experience in providing direct services related to the treatment and recovery of persons afflicted with medical issues with a particular focus on housing placement and one year of supervisory experience in a behavioral health setting; or an appropriate equivalent of the above requirements.

    Bilingual in Spanish preferred.

    Nurse Practitioner or Registered Nurse is preferred.
