**Join Our Team as an Outpatient/Inpatient Social Worker****Job Description:**

- As an Outpatient/Inpatient Social Worker, you will be the champion for those in need, providing direct supervision and supportive contact to empower clients to achieve self-sufficiency.
- Your expertise will shine as you assess client needs, design care plans, and coordinate vital services.
- Collaborate with a multidisciplinary team to:
- Assess care plans and monitor patient progress.
- Propose and implement outpatient plans for safe discharges.
- Interact with diverse individuals, from newborns to geriatric patients.
- Utilize your knowledge of community resources to link clients with social services, health care providers, substance abuse, and mental health resources.
- Engage in case reviews to ensure efficient and effective utilization of healthcare resources, promoting quality care and enhanced patient self-care.
- Foster a seamless continuum of care by working closely with providers, nurses, and case managers.
- Your role may involve navigating the complexities of mental health, providing essential support to individuals striving for improved well-being.
**Necessary Qualifications:**
- Education:
- Master's degree in social work (MSW).
- License/Certification:
- Must have a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license.
- Must maintain current Basic Life Support (BLS) certification.
- Experience:
- Minimum of three (3) years in a clinical care setting.
- Familiarity with discharge planning, case management, or utilization review processes.
- Proficient in electronic health record systems.
**Shift Information:**
- 8-hour shifts, 5 shifts per week.
- Days will vary based on staffing needs.
- Weekend rotation may be required.
