    Responsibilities of a Home Health RN:
    As a Home Health RN, you will:
    • Provide home-based care to patients post-hospital discharge
    • Collaborate with physicians and supervise other healthcare team members
    • Conduct patient assessments and document progress
    • Educate patients and families on care plans
    • Ensure patient goals are met and coordinate discharge

    Requirements:
    Key qualifications for Home Health RNs:
    • Graduate of an accredited RN program
    • Active RN license in South Carolina
    • Minimum one year of nursing experience, preferably in home healthcare
    • Med/Surg or Critical Care / IV Therapy experience is a plus
    • Clinical proficiency, communication skills, and compassion
