The Case Manager – Jail Continuity of Care will screen, administer assessments, and provide referrals to other MHMRCV programs and appropriate community resources prior to the inmate’s release.

Qualifications
- Bachelor or advanced Degree from an accredited college or university with a major in social, behavioral, or human service field including psychology, social work, sociology, human development, criminal justice.
Responsibilities
- Meet with each program participant weekly.
- Work closely with jail staff and community organizations.
