    Description

    POSITIONSUMMARY: Responsible for managing,coordinating and integrating all Care Coordination services withinCHRISTUS St. Vincent Health System. Provides CareCoordination services to patients based on person-centered acuityand coordinates patient in hospital or clinic setting. Contributesto a cooperative and accountable working relationship with othermembers of the Care Coordination system staff toward the goal ofproviding continuous high quality services to patients. Promotes optimal person-centered care that supports and empowersindividuals, respects individual choices and meets health careneeds of patients. Works side-by-side with clinical leadersin the development and implementation of protocols. May workin traditional clinic setting and/or in a training environment(residency program) or other areas as assigned.

    Requirements

    MINIMUMQUALIFICATIONS:

    EDUCATION: RegisteredNurse, BSN in Nursing,preferred.

    CERTIFICATION/LICENSES: Current New Mexico Nursing License

    SKILLS:

    • Considerable knowledge of health carefacilities, policies and issues, and of the full spectrum ofcommunity patient care services
    • Considerableknowledge of health care and social managementprinciples
    • Ability to coordinate variousfunctions and activities for maximum cooperation and integration ofservices providers and persons receiving care within a clinic forcontinuum of care services
    • Ability to direct,instruct and advise staff, and to receive and effectively react today-to-day problems
    • Ability to utilize strongcommunication skills, both written and oral, and effectivelydemonstrate an interactive style of care coordination
    • Ability to understand and implement sensitivity andculture of care measures appropriate to a diversepopulation
    • Ability to work with various levelsof the CHRISTUS organization including clinical, financial,administrative and medical staff.

    EXPERIENCE: Minimum of twoyears of experience in a health care setting and related experiencewith quality, care coordination and population healthpreferred.

    NATURE OFSUPERVISION:

    -Responsible to: Manager ofPopulation Health, Director of Ambulatory Patient CareServices

    ENVIRONMENT:

    -Bloodbornepathogen: Multiple settings and variableunpredictable situations including inpatient, specialty and primarycare settings

    PHYSICALREQUIREMENTS: Ability to move around settings ofcare. Requires close work, good vision, dexterity to write aswell as to use computer keyboard. Ability to handle stressfulsituations. Most possess and demonstrate excellent customer serviceskills.
