    Lifetime Vitality is seeking a dynamic Care Coordinator for one of our onsite contracts. Must be a bilingual
    • Spanish /English Medical Assistant The Care Coordinator is a licensed or certified medical assistant professional, who supports the care management of patients receiving Remote Physiological Monitoring, Chronic Care Management, Transitional Care Management and/or Behavioral Health Integration.
    The Care Coordinator serves assigned medical practices as clinical staff for physician groups contracted with Lifetime Vitality.

    Experience Necessary to perform job duties:
    â
    • At least 3 years experience as a medical assistant â
    • Virtual Care experience is strongly preferred â
    • Experience in using multiple Electronic Health Records for physician practices â
    • Experience in using Vonage or a similar VOIP system â
    • Telehealth experience strongly preferred.

    Skills necessary to perform job duties:
    â- Ability to accommodate and communicate effectively with various physicians, physician"'s assistants and nurse practitioners â- Exceptional written and verbal communication skills â- Competent in using multiple electronic health records that support the suite of Lifetime Vitality Providers across the country â- Proficient with google suite/Microsoft office â- Deep understanding of Social Determinants of Health and Health Literacy and the ability to modify auditory-verbal communication over the phone to meet the needs of our patients across the nation â- In depth knowledge of Medicare and Private Health Insurer requirements for CCM, RPM, TCM and BHI

    Daily job duties:
    â- Works on the consenting team, training team, care management and monitoring teams for virtual care services â- Understands compliance for all virtual service CPT codes and works to achieve necessary compliance for each patient assigned â- Supports physicians for E/M telehealth visits and development of care plans â- Reports care management concerns via to the immediate supervisor or specified physician/provider preferred triage methods â- Other duties as assigned and necessary for the success of the organization

    • Weekends and Evenings required as assigned and needed for patient management and provider preference â
