Job summary
The Transfer Center Specialist II (LPN, Paramedic, EMT) will work remotely and coordinate through inbound and outbound calls the acceptance of patients into LifePoint Health system and facilitates patient transfer process with patients going out of LifePoint Health system to non-LifePoint health system.This is a Remote, Full-Time opportunity.
We are looking for experienced LPN, Paramedic or EMTs to perform the following duties:
- Performs initial admission screening and acceptance using approved clinical criteria to assure patient acceptance and admission are consistent with current protocols and procedures.
- Works in collaboration with multiple departments within the hospital and facilitates relationships with facilities, physicians, and nursing staff.
- Works closely with the Bed/Capacity Coordinator to identify, trend
and report barriers to patient throughput. The position works closely
with other Transfer Center Advocates to support 24/7 operations.
