    Description
    RN will perform the following work for A&C Private Home Care (the "Scope of Work"):
    a. The Registered Nurse shall supervise the personal care, companion, and sitter services offered by A&C Private Home Care.
    The RN will be the primary supervisor of personal care tasks for clients determined to be medically frail or medically
    compromised. The RN shall supervise the provision of personal care tasks in accordance with accepted standard care.
    b. The RN will assess physical and mental health needs, monitor vital signs as needed, observe the functional levels of the
    client and note any changes in the physical condition in the client's record/chart. She/he will teach and encourage self-care;
    coordinate care plans with physician and other service delivery agencies; supervise the development and implementation
    of the provide care plan. And supervise personal care tasks for clients determined to be medically frail or medically
    compromised.

    Standard Duties Includes:
     Client evaluation
     Implementation of service plan
     Preventive and rehabilitative nursing procedures
     Clinical progress notes preparation
     Coordination with other service providers
     Notification of physician and case manager of changes in the client's condition or additional needs
     Health related education to meet client needs
     Supervision and teaching other personnel
     Coordination of all specialized therapies and supervise any services provided for medically frail clients

    The work will be completed upon review and approval by A&C Private Home Care, which approval shall not be unreasonably
    withheld.
    Change Orders
    If A&C Private Home Care makes any changes to the above Scope of Work, it must be through a written "Change Order" signed by
    both parties detailing the additional work to be performed and any associated costs and fees.

    Payment
    A&C Private Home Care will pay Contractor: (the checked term applies)
     a fee of $____________50_________________ per Visit
     a mileage reimbursement rate of $.31 per mile. Both fee and milage will be paid in the next payroll period upon completion
    of the work.

    RN will document all the work and provide the documentation to and invoice A&C Private Home Care for amounts payable.


    The checked term applies:
     RN will be solely responsible for the cost of materials used for work performed under this agreement.
     A&C Private Home Care will reimburse, with prior approval, RN's mileage for work performed under this agreement.
    Independent Contractor Relationship
    Unless otherwise required by law, RN is an independent contractor, not an employee of A&C Private Home Care, and is solely
    responsible for all taxes incurred in connection with this agreement. A&C Private Home Care will not withhold on RN's behalf FICA
    (Social Security and Medicare taxes) or state or federal income tax, and will not make state or federal unemployment contributions on
    RN's behalf, unless required by law.
    Permits, Certificates & Licenses
    RN will be solely responsible for determining Georgia state and local permits, certificates and/or licenses, if any, are necessary for
    undertaking and completing the work, and for obtaining those permits, certificates and/or licenses. Copies of permits, certificates
    and/or licenses shall be provided to A&C Private Home Care for record purposes.
    Insurance
    RN will maintain adequate insurance for incurring loss or injury due to the acts of RN, its employees or subcontractors in connection
    with this agreement.

    LIMITED WARRANTY
    RN WARRANTS THAT THE WORK UNDER THIS AGREEMENT WILL BE COMPLETED IN A TIMELY AND WORKMANLIKE MANNER MEETING
    OR EXCEEDING THE STANDARDS FOR SIMILAR WORK IN CLIENT'S COMMUNITY OR REGION AND WILL COMPLY WITH ALL APPLICABLE
