    **Description**

    A first responder from our agency who is passionate about the services we offer, and works directly with patient/family/authorized representative, or any other customer to insure optimal access. Keeps patient centered services and caring at the forefront, exceeding customer expectations and insuring best practice and flawless care in all aspects

    JOB DUTIES/RESPONSIBILITIES:

    1. Quality

    a. Provides for optimal access to Empath Health/Suncoast Hospice services.

    b. Responds to customer concerns in a professional manner that supports Empath Health/Suncoast Hospice mission.

    c. Facilitates rapid, thorough access to Empath Health/Suncoast Hospice Integrated Network of Care.

    d. Possesses the skills to educate about and conduct personal interviews with patients/families/authorized representatives while successfully articulating services.

    e. Maintains and continually builds rapport and trust with our community partners, community agencies, physicians, hospitals, and nursing homes.

    f. Completes required documentation in real time by being accurate, thorough and detail-oriented from referral through admission.

    g. Is able to anticipate problems before they become complaints and/or resolve complaints effectively, considering every comment or complaint as an opportunity to learn and to prevent the same problem from arising again.

    h. Ability to work alongside the RN in all settings, while traveling to where the patient is.

    i. Seeks every opportunity to meet and exceed customer needs and satisfaction.

    2. Promotes social and emotional well being.

    a. Maintains patient and family confidentiality at all times.

    b. Able to assist with inquiries/education regarding self-determined life closure issues (Advance Directives), while also being knowledgeable about FL Proxy Guidelines to insure proper signage of all forms related to admission process.

    3. Stewardship

    a. Continually acts as an ambassador for Empath Health/Suncoast Hospice when communicating with all customers.

    4. Teamwork

    a. Contributes successfully to the development of a high functioning team.

    b. Participates and promotes the agency strategic goals related specific to access, customer service and quality.

    5. Professional Development

    a. Consistently updates knowledge of Empath Health/Suncoast Hospice programs.

    b. Performs a self evaluation of strengths and weaknesses annually.

    6. Other duties as needed by Care Navigation Leadership.

    Positions Qualifications/Requirements:

    Education and/or Experience:High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
