    Description


    Seeking a full-time and/or part-time RN position to provide skilled nursing services to home care clients in the Twin Cities metro area.

    All nurses will work collaboratively with a team of Professionals to promote health and safety of clients in their homes.

    . No weekends or evenings required.


    PATIENT CARE
    Provide skilled nursing care to patients in their homes

    Conduct comprehensive assessments of patients' health status

    Develop and implement individualized care plans

    Administer medications and treatments as prescribed by physicians

    Monitor and evaluate patients' progress and adjust care plans as needed

    Educate patients and their families on proper healthcare techniques and disease management

    Collaborate with other healthcare professionals to coordinate patient care

    Document all patient interactions, interventions, and outcomes accurately and timely

    Monitor and report changes in patient symptoms or behavior

    Maintain accurate patient medical records


    OTHER DUTIES
    Completes, maintains, and submits accurate and relevant clinical notes regarding patients condition and care given. Records pain/symptom management changes/outcomes as appropriate.

    Communicates with the physician regarding the patients needs and reports changes in the patients condition; obtains/receives physicians orders as required.


    Communicates with community health related persons to coordinate the care plan.

    Teaches the patient and family/caregiver self-care techniques as appropriate.

    Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.

    Works in concert with the interdisciplinary group.

    Provides and maintains a safe environment for the patient.

    Assists the patient and family/caregiver and other team members in providing continuity of care.

    Works in cooperation with the family/caregiver and homecare interdisciplinary group to meet the emotional needs of the patient and family/caregiver.


    Attends interdisciplinary group meetings.

    Be able to schedule and maintain visits on scheduling software (eRSP)

    Part-time (negotialbe)


    Salary:
    $ $50.00 per hour

    Mileage Reimbusment (added to hourly pay rate)

    Schedule :
    Manditory 2 hours in office on Wedensdays

    You will work independently in the community providing one-on-one care.

    Visits generally are scheduled between 8am - 5pm.

    Ability to commute

    Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.


    Twin Cities/Minneapolis area


    Experience:
    Graduate of an accredited school of nursing.

    One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care preferred.


    Current licensure in State.
