- Responsible for patient assessment and transitions of care into ABCM Corporation care centers
- Conducts preadmission and readmission assessments and obtains medical record documents to meet regulatory requirements for continuity of care. As part of this process, the Corporate Care Liaison meets with potential admissions and their involved family members to provide care center information, explain services, assist in determining payment sources/finances, and answer questions as needed
- Maintains regular contact with Iowa acute care health centers and ABCM care centers regarding anticipated skilled nursing facility/long-term care discharges
- Communicates with care center nursing staff regarding potential resident needs and requests, and ensures the facility is aware of the necessary supplies and equipment required to meet the needs of the potential admission
- Works to build relationships, educate, and market to referral sources
- Must have either an unrestricted and active Iowa (or compact state) RN or LPN license or a BA in Social Work or Human Services (work experience can compensate for education)
- Experience in long term care (LTC) and/or acute care and knowledge of Medicare and Medicaid preferred
- Must be compassionate and detail-oriented, along with possessing excellent communication and interpersonal skills
- Must be willing to work flexible hours, including weekend and on-call rotations
- Reliable transportation/travel required
- Per the Centers for Medicare and Medicaid Services (CMS), COVID-19 vaccination is required, with exceptions for medical and religious accommodations when reasonable
