Job summary
The Clinical Pharmacist is responsible for delivering comprehensive medication management services including targeted medication reviews therapeutic drug monitoring patient education and counseling drug therapy consultations medication reconciliation clinical in-services and education programs. This role supports the Ambulatory Pharmacy Services and Patient Management Program to ensure safe effective and patient-centered care.- Perform all key clinical pharmacy responsibilities including therapy management monitoring patient support medication access and education referrals prior authorizations and medication reconciliation.
- Analyze verify accuracy appropriateness safety of prescribed therapy.
