- 12 hours shifts (7:30am - 8:00pm)
- Every 3rd weekend rotation and 2 holiday's a year
- Assists with the implementation of daily program activities for behavioral health patients
- Provide support for the patients' emotional and social needs
- This position also monitors and identifies anything needed to promote safe patient care

- Bachelor's degree in Social Work or Psychology or Sociology or a related human service field
- 1 year experience working in human services field or health care setting
- Provides developmentally appropriate supportive interaction with counseling and monitoring of patients in a 1:1 or group setting.
- Assess changes in patient's behavior and notifies the RN.
- Assists the treatment team and partner with patient in developing and modifying Behavioral Health Plan of Care for patients.
- Partners with the treatment team to encourage patient compliance with Behavioral Health Plan of Care and participation in activities.
- Assists RN with admission, transfer, and discharge process.
- Facilitates educational groups.
- Participates in family meetings.
- Documents in EPIC.
- Demonstrates role accountability by adhering to Gundersen Health System statements, policies, and confidentiality statement.
- Performs other job-related responsibilities as requested.
