Job Description

Basin Home Health is seeking a compassionate and dedicated Physical Therapist to join our team. As the leading home healthcare provider in northwest New Mexico, we have delivered exceptional, personalized healthcare services in clients' homes since 1992.

Responsibilities:

Assess client needs and develop treatment plans.
Collaborate with multidisciplinary teams to implement comprehensive care plans.
Monitor, document, and adjust clients' progress as needed.
Educate clients and caregivers on home exercise programs and safety.
Adhere to regulations and company policies; participate in ongoing professional development.

Requirements:

Current state licensure as a Physical Therapist
Graduation from an accredited Physical Therapy program
Home health experience is preferred but not required.
Proficient in electronic medical records and documentation
Valid driver's license, reliable transportation, and CPR certification
Background check and drug screen clearance

* $20,000 sign-on bonus terms: Bonus to be paid out in quarterly increments over the first year of employment with our agency. Candidate must be full-time to qualify for the sign-on bonus and other benefits. Note that if you resign your employment, are terminated for cause, or fail to complete one full year of full-time employment, you will be responsible for repaying the sign-on bonus to the agency on a pro-rata basis.
