    St. JACINTA was created because Catholic Charities NH has responded to the era of Agency-centric staffing. We understand that staff want flexibility for a variety of reasons.

    JACINTA staff get the flexibility they want with a per diem schedule, and the benefit of choice in a network of 7 long term nursing care facilities in New Hampshire:

    Manchester - Windham - Laconia - Dover - Berlin.

    Long Term Care positions are currently per diem up to four 12-hour shifts a week

    Requirements:

    Rn license required

    Interact professionally with residents, physicians, family members, CCNH medical staff, visitors and government agencies.


    • Respond quickly and with poise to emergencies


    • Function with minimal supervision


    • IV certified or intention to be


    • CPR/BLS certified

    The Licensed Practical nurse maintains ultimate responsibility for the delivery of resident care during a specific shift on a designated unit.


    • Maintain awareness of care plans


    • Conduct clinical rounds on assigned unit several times during the shift


    • Maintain confidentiality of all resident related care information to ensure HIPAA compliance


    • Confirm the identity of residents prior to providing care, treatments, medications or serving meals


    • Monitor and ensure completion of LNA flow sheets


    • Recognize emergency clinical situations, assess immediate needs , implement appropriate intervention and communicate situation to the nursing supervisor


    • Immediately report all medication errors and sentinel events to the nursing supervisor


    • Ensure that all medications and controlled substances are administered, documented and stored in compliance with pharmacy and facility policies


    • Attain appropriate CEUs to maintain LPN Licensure


    • Perform all duties in a safe, non-hazardous manner

    Nurses must be flexible and reliable when they maintain Agency employment, as they are whom an organization relies on to support regular employee leaves, vacations and time off.
