    Description

    Provide direct personal care to residents following the individualized service plan (ISP) to maximize their comfort and quality of life.


    Principle Accountabilities and Responsibilities:
    Northern Lights Services, Inc considers the following principal accountabilities to be the essential functions of the position. Successful job applicants and incumbents will be able to perform these essential functions with or without requested reasonable accommodation.


    • Provide quality care and services for all residents designated in the ISP in a dignified and respectful manner to allow each resident to function at his or her highest level of independence. Includes assistance with dressing, grooming, bathing, eating, toileting, meal preparation, housekeeping, laundry, medication management and other duties to benefit the residents.
    • Provides care following the ISP and reports to the team any change in behavior or condition that may determine needed changes to the ISP.
    • Report to The Oaks Director on the current condition and needs of the resident, The Oaks Director will update the resident's families or guardian unless it is an emergency situation.
    • Read and contribute to communication book and speak to The Oaks Director regarding any care needs of specific residents or information that is beneficial to the operations of the facility.
    • Assist other team members in providing care for residents.
    • Attend required trainings and in-service sessions to meet the required training for certification and the requirement of the facility.
    • Participate in team meetings and committees designed to improve and maintain the facility.
    • Perform all duties and responsibilities in accordance with infection control and safety guidelines as defined by facility, state, and federal regulations.
    • Become CPR certified.

    Knowledge, Skill and Abilities:

    • Experience in the healthcare field is preferred but not required. High school diploma or GED preferred.
    • Within 90 days, must have met all state required certifications per DHS 83.20.
    • Requires ability to positively relate to the needs of the CBRF clients and have a genuine appreciation and interest in older people and those with special needs.
    • Must be able to speak, read, and write English in a proficient and understandable manner.
    • Must be able to physically perform all duties on physical profile form.
    • Must be willing and able to make independent decisions in the best interest of the organization.
    • Requires patience, tact and willingness to work with and communicate effectively with people regardless of the level at which they are currently functioning.
    • Must be able to work effectively and politely with the public under all conditions.
    • Must meet the requirements of the criminal background check law.
    • Regular and punctual attendance is expected and required.
