Diploma or Equivalent Required andOther Successful completion of a Community Health Worker formal training program such as from a community college or other education institution PreferredExperience6 Months Customer service experience either face-to-face or by phone RequiredLicensures and CertificationsBCLS - Basic Cardiac Life Support within 60 days RequiredSkillsStrong verbal and written communication skills, ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented, ability to work independently as well as with others, strong organizational & prioritization skills, and ability to motivate and inspire others, ability to complete required reports.
- Responsible for establishing trusting relationships with patients and families.
- Assists patients with personal goal setting and care plans.
- Supports clients with meeting personal goals including but not limited to attending appointments, providing support for scheduling meetings/appointments with agencies and resource providers. Efforts will support development of self-management skills that improve health and well-being.
- Participate in program development, planning and evaluation.
- Develop and effectively manage program timelines to ensure timely completion of program deliverables.
