    *Job Description: *As a Nurse Practitioner in our assisted living facility, you will play a crucial role in the delivery of high-quality healthcare services to our adult residents. You will work closely with our interdisciplinary team, which includes physicians, nurses, and other healthcare professionals, to ensure comprehensive and holistic care for our residents.

    *Job Qualifications:
    • Master's or Doctorate degree in Nursing (MSN or DNP) from an accredited program.
    • Current and unrestricted Nurse Practitioner license in NY
    • Certification in Adult, Family, or Rehab specialty from a nationally recognized certifying body.
    • Minimum of 1 year of experience as a Nurse Practitioner, preferably in adult, family, or rehab settings.
    • Experience working in an assisted living facility or similar long-term care environment is a plus.
    • Solid understanding of geriatric care principles and best practices.
    • Excellent communication and interpersonal skills, with the ability to work effectively within a team.
    • Strong critical thinking and problem-solving abilities to address complex medical situations.
    • Proven ability to manage time efficiently and prioritize tasks effectively.
    • Willingness to embrace our facility's mission and values, promoting a culture of compassion and respect.
    *Responsibilities include but are not limited to:
    • Conduct comprehensive health assessments for adult residents with a focus on adult, family, or rehab specialties.
    • Collaborate with the interdisciplinary team to develop individualized care plans and treatment goals for residents.
    • Monitor residents' health status, evaluate responses to treatments, and make necessary adjustments to care plans.
    • Prescribe medications
    • Perform minor medical procedures and wound care as needed.
    • Educate residents and their families about medical conditions, treatment plans, and self-management strategies.
    • Provide support and guidance to nursing staff in managing complex medical cases and resident care issues.
    • Assist in coordinating and conducting staff training on relevant medical topics and best practices.
    • Collaborate with external healthcare providers, specialists, and hospitals as needed.
    • Participate in facility-wide quality improvement initiatives to enhance resident care and safety.
    • Adhere to all legal and ethical standards of nursing practice, ensuring confidentiality and privacy of resident information.
    • Maintain accurate and up-to-date documentation of all patient encounters, treatments, and medical records.
    • Foster a positive and caring environment for residents, families, and staff
