    Description


    JOB DESCRIPTION:

    • Responsible for enhancing the patient experience by providing a seamless navigation process from diagnosis to survivorship.
    • Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating, providing a link to research
    • Serving as the primary point of contact for patients and families and providing indirect patient care.
    • Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.

    MINIMUM QUALIFICATIONS:
    Graduate of an accredited school of nursing; BSN required. Two years nursing experience required, 1 year case management preferred. BLS required.

    (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.
