    Communities of Care is in search of a Private Duty Nurse to join our team of experienced nurses who play a vital role in the healthcare field.


    • Seek individuals for 12-hour shifts from Monday to Friday in Golden Valley.
    • Patient profile: 13-year-old with tracheostomy, ventilator, gastrostomy tube, and nonverbal
    We provide top-notch care while embracing the values of a close-knit community.

    We will provide comprehensive training.


    Responsibilities:

    • Administer healthcare to patients who are ill, injured, or disabled
    • Identify and develop patient care plans
    • Monitor and report any changes in patient conditions or behavior
    • Collaborate with physicians and specialists on patient treatment
    • Educate patients on maintaining good health and preventing diseases
    • Arrange referrals to other medical professionals and facilities
    • Keep precise medical records of patients
    • Offer guidance and emotional support to patients and their families

    Qualifications:

    • Proficiency in using medical tools and software
    • Ability to establish a connection with patients
    • Strong problem-solving and critical thinking abilities
