    Responsibilities:

    • Deliver direct patient care within the scope defined by the state Nurse Practice Act
    • Execute care plans developed by registered nurses
    • Ensure accurate and timely documentation aligned with the care plan
    • Educate patients and families on self-care practices and relevant information
    • Participate in coordinating home health services and communicating needs to other healthcare disciplines
    • Handle equipment and supplies following aseptic protocols
    • Engage in personal and professional development
    • Perform additional tasks delegated by the registered nurse
    • Provide healthcare guidance based on assessments and care plans

    Requirements:

    • Graduate of an accredited practical or vocational nursing program
    • Minimum of three years of nursing experience, with preference given to community/home health or medical/surgical background
    • Active LPN or LVN license in the state
    • Valid CPR certification
    • Reliable transportation, valid driver's license, and appropriate car insurance
