    Role Description


    • Promote positive patient outcomes through coordination and sequencing of care services from pre-admission through post-discharge stages.
    • Serve as a client advocate by identifying issues within healthcare delivery systems.
    • Maintain open communication channels among interdisciplinary teams, agencies, stakeholders including managed care organization case managers involved in treatment plans.
