ABCM Corporation - Oakwood Care Center and The Courtyard Independent & Assisted Living ) Clear Lake, IA $21.00 to $22.50 per hour
Summary
Under the supervision of the Administrator:
+ Utilizing knowledge of the casework service process and employing associated techniques, [the Social Worker] interviews, counsels, assesses, and intervenes (as needed), in order to affect positive change in the elderly / disabled resident's emotional, behavioral, and psychosocial functioning
+ Accesses / coordinates appropriate treatment and services for resident psychosocial adjustment difficulties and effectively communicates with team members, residents, resident families, legal representatives, and community resources accordingly
+ Ensures that the corporate / care center social services policies and procedures for pre-admission, admission, advocacy, direct / indirect counseling, discharge, and community liaison services are in compliance and implemented in a timely and efficient manner
Qualifications / Requirements
+ Bachelor's degree in social work or human services field, including but not limited to: sociology, special education, rehabilitation counseling, or psychology
+ Must have a minimum of one (1) year of supervisory social work / human services experience in a healthcare setting, working directly with individuals similar to those who reside in our communities
+ Must be knowledgeable of community resources and support services
+ Per the Centers for Medicare and Medicaid Services (CMS), COVID-19 vaccination is required, with exceptions for medical and religious accommodations when reasonable

Job Type: Full-Time
Schedule: Days; appx. thirty-seven (37) hours/wk
Wage Range*: $21.00/hr-$22.50/hr, Negotiable (DOE)
