    About the Company:

    Communilife is seeking a dedicated Substance Abuse Counselor for its latest program, Brooklyn House. As part of the Comprehensive Case Management (CCM) Team, you will play a crucial role in providing services to formerly homeless individuals struggling with mental health and substance abuse issues.

    Job Description:

    Communilife is looking for a skilled Substance Abuse Counselor to join the DHS program at Brooklyn House. This position involves conducting assessments, developing treatment plans, and providing support services to formerly homeless individuals with co-occurring mental illness and substance abuse disorders. The services will be based on evidence-based treatment practices to ensure the well-being of the residents.

    Key Responsibilities:

    • Provide enhanced and expanded treatment for clients with substance abuse disorders
    • Conduct assessments to identify care needs and make appropriate referrals
    • Deliver individual, family, and group counseling sessions
    • Facilitate psycho-education groups related to health and wellness
    • Assist clients in achieving functional improvements and housing stability
    • Participate in discharge planning and follow-up services
    • Maintain accurate client case records and documentation

    Qualifications:

    This role requires a Bachelor's Degree in a human services field and at least three years of experience working with clients with substance abuse disorders. A CASAC certification is mandatory. Experience with mentally ill individuals and bilingual (English/Spanish) skills are preferred. Strong communication and computer skills are essential for this position.
