Job summary
We are seeking a skilled Clinical Pharmacist to join a high-acuity inpatient pharmacy team serving ED, ICU, NICU/PICU, oncology, cardiac surgical and general medical populations.Responsibilities
- Review and verify medication orders to ensure accuracy and clinical appropriateness
- Compound dispense medications following USP state and facility standards
- Evaluate therapies for allergies contraindications interactions and dosing accuracy
- Provide pharmacokinetics monitoring anticoagulation management TPN nutrition support
- Perform renal hepatic dose adjustments IV-to-PO conversions
