- Receive hospice referrals by phone, fax, secure text or in person.
- Ensure smooth intake operations and data gathering for all referrals.
- Provide clear communication of hospice services and eligibility requirements to patients, families and community contacts.
- Develop and implement Initial Plan of Care with input from patient, caregiver, physician, IDG and other personnel as needed.
- Conduct comprehensive nursing assessment including physical, emotional, and mental needs.
- Assist patients, family members or other clients with concern and empathy; respect confidentiality and privacy and communicate in a courteous and respectful manner.
- Ensure that arrangements for medication, equipment, services and supplies are communicated and provided in a timely manner.
- Flexibility in work schedule to accommodate patient needs, including new admissions.
- Performs other duties as assigned.
- Current Missouri Registered Nurse license in good standing.
- Working knowledge of hospice regulations preferred.
- Excellent organizational and time management skills.
- Excellent communication skills with a multidisciplinary team of clinicians, clerical staff, and management.
- Participate in IDG/care conferences and in-services.
- Strong clinical judgment, assessment, communication, and leadership skills.
- Proficient in computer skills and technology to complete documentation of clinical records.
- Must have dependable transportation and a current driver's license.
- Will work weekends and holidays as needed.

Job Summary:
The Admission Registered Nurse (RN) admits patients to hospice service in accordance with all guidelines and regulations. This role will coordinate with RNs and other hospice staff to establish care for all incoming patients in a prompt manner. For this role, you need to be a registered nurse that has a passion to improve the quality of life for terminally ill patients with a goal of helping patients live their final days with dignity and comfort.
