    Community Support Specialist

    REPORTS TO: Director of Residential Services

    POSITION DESCRIPTION

    Community Support consists of specific activities with or on behalf of a particular client in accordance with an individual rehabilitation plan to maximize the client's adjustment and functioning within the community while achieving sobriety and sustaining recovery, maximizing the involvement of natural support systems, and promoting client independence and responsibility.

    Job Responsibilities

    1. Develop recovery goals and identify needs, strengths, skills, resources and
    supports and teaching how to use these supports, identify barriers to recovery

    2. Assist in the development of treatment plans

    3. Assist clients in develop natural supports to achieve and maintain recovery

    4. Help clients restore skills and resources that were negatively impacted by
    substance use

    5. Participate in the interdisciplinary team meeting in order to identify strengths and
    needs related to development of the individual's rehabilitation plan including but
    not limited to:

    a. Employment;

    b. Education;

    c. Housing;

    d. Nutrition, meal preparation, meal planning and shopping

    e. Lifestyle changes and healthy lifestyle education

    and actively work with clients to complete treatment goals and objectives

    6. Attend periodic meetings with designated team members and the client,
    whenever feasible, in order to review and update the rehabilitation plan

    7. Contact clients who have no showed within 72 hours in order to re-engage the
    person and promote recovery efforts

    8. Arrange and refer for services and resources and, when necessary advocating
    obtaining the services and quality of services to which the person is entitled

    9. Monitor service delivery by providers external to the program and ensuring
    communication and coordination of services

    10. Locate and coordinate services and resources to resolve a crisis

    11. Provide experiential training in life skills and resource acquisition

    12. Provide information and education to an individual in accordance with the
    person's rehabilitation plan

    13. Plan for discharge

    14. Consistently meet monthly billable expectations

    15. Transport clients in employee's vehicle

    16. Provide group education

    17. Ability to lift 20 pounds

    18. Assist client in establishing treatment goals

    19. Schedule includes one weeknight per week and one weekend coverage per
    month

    20. Other duties as assigned

    Job Requirements

    1. Bachelor's degree in social work, counseling, psychology, or closely related field;
    or

    a. A combination of higher education and qualifying experience equaling four (4)
    year; or

    b. Associate of Applied Science in Behavioral Health Support degree from an
    approved institution; or

    c. Four (4) years of qualifying experience

    2. Valid Class E (or other acceptable) driver's license, vehicle, and current automobile
    liability insurance

    3. Current and annual TB and CPR

    Job Type: Full-time

    Pay: From $37,000.00 per year
