- Conduct annual home health risk assessments for a Medicare Advantage health plan
- Evaluate prior and current medical history, medical conditions and medication history
- Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
- Provide education on medical conditions and medications
- Flexible shifts between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
- 6 months of current NP experience is preferred
- Nurse Practitioner License in NC
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited school of nursing

Job DescriptionMinimum 1+ years nurse practitioner experience required.There is an open Per Diem Nurse Practitioner (NP) Home Health Flexible Schedule job in Roanoke Rapids, NC. The Per Diem Nurse Practitioner (NP) will conduct home health risk assessments within Halifax County, NC. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Roanoke Rapids, NCand the surrounding Halifax County, NC.Halifax County, NCincludes locations like Littleton, Scotland Neck, and Weldon.Candidates could be scheduled in any one of these locations during an individual shift.
