    Job Description:

    This role involves the following responsibilities:

    • Advocate for patient needs through continuous psychosocial assessments and communication with the treatment team.
    • Collaborate with the treatment team to plan and execute suitable inpatient dispositions.
    • Identify patient, family, institutional, and community barriers to discharge or treatment transitions. Coordinate referrals and facilitate treatment transitions while addressing these barriers.
    • Coordinate the creation and implementation of psychosocial and case management assessments using community resources.
    • Provide or facilitate direct clinical therapeutic interventions to prevent relapse and ensure care continuity based on physician care plans.
    • Offer information and education to patients and their families/significant others regarding illness, recovery, and treatment options.
    • Collaborate professionally with the treatment team to maintain a relationship with external providers and the reimbursement team for optimal patient care provision.

    Qualifications

    • Master's Degree in Social Work required with a valid LICSW.
    • Demonstrated proficiency in:
      • Individual case management and knowledge of managed care.
      • De-escalation/restraint management techniques.
      • Motivational Interviewing and short-term treatment techniques.
      • Assessment and development of treatment plans to meet patient needs.
      • Strong organizational skills and adaptability to a fast-paced work environment.
      • Conducting brief family interventions as necessary.
