    Avera Medical Group Palliative Medicine Yankton is seeking a compassionate and dedicated Social Worker (MSW) to join our team in providing essential support in the Palliative Care setting.

    The ideal candidate will have a Master's degree in Social Work from an accredited program, be licensed as a Social Worker in the State of South Dakota, and possess excellent communication and interpersonal skills.

    This role involves addressing the emotional and social needs of patients and families facing chronic illness, developing individualized care plans, facilitating referrals to community resources, and delivering clinical therapeutic services as part of a multidisciplinary team.

    Key Responsibilities:
