    W e are working with an amazing client who is seeking an energetic, visionary, and growth-oriented Director of Nursing (DON) to lead their Skilled Home Health division.


    This pivotal leadership role combines clinical excellence, operational strength, and business development acumen, ensuring the highest level of patient care while driving organizational growth and maintaining full compliance in preparation for our ACHC accreditation survey (February 2027).


    The ideal candidate will have an understanding of Medicare-certified Home Health operations, a passion for building strong teams, and a track record of leading clinical programs to sustained success.

    This is a leadership-level position designed to build infrastructure and offers the opportunity to shape the future of their Skilled division.

    Key Responsibilities Clinical Leadership & Compliance Oversight
    Serve as the Director of Nursing for our Medicare-certified home health operations.
    Ensure full compliance with Medicare Conditions of Participation and ACHC accreditation standards.
    Oversee OASIS accuracy, timeliness, and quality assurance.
    Monitor Plans of Care, physician orders, and timely NOA and billing submissions.
    Lead QAPI (Quality Assurance and Performance Improvement) initiatives.
    Conduct periodic internal chart audits, mock surveys, and maintain continuous survey readiness.

    Skilled Operations Management
    Oversee patient census, episode tracking, and intake-to-admission workflow.
    Supervise and mentor field clinicians (RN, PT, OT, ST) while reviewing documentation for compliance and quality.
    Partner with administrative leadership in weekly revenue, compliance, and process improvement meetings.
    Identify workflow bottlenecks and implement operational efficiencies to support scalable growth.

    Growth, Referral Development & Community Engagement
    Build and maintain referral relationships with hospitals, physician offices, discharge planners, and social workers.
    Strengthen internal cross-referral processes between HVHC's service lines.
    Support community outreach, education, and partnership development to drive sustained census growth.
    Represent HVHC professionally within the local healthcare community to expand our agency's clinical reputation and reach.

    Accreditation & Continuous Readiness
    Lead ongoing preparation for the February 2027 ACHC survey.
    Maintain organized compliance documentation, personnel files, and training records.
    Conduct staff preparedness sessions, internal tracer reviews, and policy updates as needed.

    Field Responsibilities (Limited & Structured)
    Conduct skilled nursing visits as clinically necessary.
    Maintain a light field load to focus on leadership and operational objectives.
    Support additional field RN hiring as census expands.

    Qualifications
    Active Registered Nurse (RN) license in the State of Pennsylvania (required).
    3–5+ years of experience in a Medicare-certified Home Health environment preferred.
    Prior DON, Clinical Supervisor, or leadership experience preferred.
    In-depth knowledge of OASIS documentation, ACHC standards, and Medicare Conditions of Participation.
    Understanding of billing, NOA processes, and compliance protocols.
    Proven ability to lead teams, manage workflow systems, and maintain regulatory excellence.
    Strong leadership, organizational, and interpersonal communication skills.

    Skills & Competencies
    Proficiency with EMR/EHR systems and healthcare documentation standards.
    Knowledge of Medicare, HIPAA, and state/federal compliance requirements.
    Skilled at care plan development, interdisciplinary collaboration, and quality improvement.
    Ability to coach, motivate, and mentor clinical staff toward high performance.
    Strategic mindset with the ability to drive growth initiatives in a regulated environment.
