Nurse Practitioner Position at Providence ElderPlace in Portland, Oregon

We are looking for a full-time nurse practitioner to join the Providence ElderPlace's Program of All-Inclusive Care for the Elderly (PACE) in Portland, Oregon. This unique program offers comprehensive care, social services, psychiatric care, and acute care to geriatric patients across the Portland Metro area. Joining our team means providing these valuable services in patients' homes and medical clinics, collaborating closely with various healthcare professionals. It's a fantastic opportunity to be part of a caring group of experts serving vulnerable members of the Portland community.

Monday to Friday, daytime hours
Outpatient primary care setting including home visits
Collaborate with physicians, nurse care manages, social workers, pharmacists, and other team members
Must be a board-certified/board-eligible Psychiatric Mental Health Nurse Practitioner
Ideal candidates with interest in obtaining MAT Waiver for prescribing Suboxone
