The Registered Nurse (RN) in Home Health delivers and oversees nursing care for patients in their homes according to physician orders and in compliance with laws, regulations, and agency protocols. Additionally, the RN job involves coordinating the overall care plan with fellow healthcare professionals and facilitating seamless patient care continuity through communication with physicians, agency staff, patients, families, and community resources. All done in a Point of Care environment.

• Delivers clinical services within the scope of practice as outlined by state regulations governing nursing practice, in line with the care plan and in collaboration with the healthcare team.
• Conducts initial and/or comprehensive nursing assessments, establishes primary care focus, develops care plans within State-specific guidelines, and maintains accurate, complete, and timely documentation as per policy.
• Regularly assesses and re-evaluates the patient's nursing requirements (at least every 60 days or as needed due to changes in condition).
• Performs comprehensive patient assessments at specified time intervals and formulates an appropriate care plan with input from physician orders.
• Verifies that patients meet home health eligibility and medical necessity criteria set by the payer source.
• Initiates, creates, implements, and adjusts care plans as necessary, in coordination with physicians and other healthcare professionals involved in patient care.

License Requirements

• Active RN license in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation

Additional State Requirements

• Minimum one year of clinical experience as a Registered Nurse. One year of clinical experience as an LPN may be considered in lieu of Registered Nurse experience for candidates with recent clinical exposure. RN license must be unrestricted.
