In this role, you will work collaboratively with clients to determine their non-medical needs, develop the best course of action, and oversee their care plan alongside the Team Lead (RN), ensuring each client gets high-quality, individualized care.

The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices. Responsibilities Collaborate with referral sources, clients, families, and Admin staff Maintain Client Secured Files Develop individualized care plans Educate clients on their home care options Create goals and monitor progress toward goals Community Liaison Active Communication with clients and families Qualifications Previous experience as a Care Coordinator or in a similar position is preferred Certification as a medical assistant is preferred Strong problem-solving and organizational skills Ability to manage multiple projects or tasks and prioritize appropriately Ability to work in fast-paced situations and make sound decisions quickly Excellent interpersonal skills and high level of compassion Strong verbal and written communication skills
