- The Senior Manager will lead a team of Nurse Practitioners responsible for assessing and evaluating the health care needs of patients, including the Annual Health Assessments, Clinical Chart Summary, supporting operations, and other programs.
- This will be a working manager role whereby the Sr. Manager may also provide the services, including in-home needs evaluations for patients.
- Lead and mentor a team of Nurse Practitioners, providing guidance, support, and training as needed.
- Collaborate with cross-functional teams, to ensure alignment with organizational goals for Care Where You Are and AHA.
- Review team performance metrics to ensure alignment with program commitments.
- Performs Comprehensive Health Assessments of patients in person and via telehealth.
- A Master's Degree in Nursing is required.
- Active RN licensure and Nurse Practitioner Certification in California.
- Active and good standing NPI number.
- Minimum 10 or more years of Primary Care nursing experience is required.
- Minimum 4 years of leadership experience is required.
- 4 years of Primary Care Nurse Practitioner experience is required.
- Possession of a current Furnishing Number.
