Summary of Program and Position:
This Clinical Case Manager provides youth- and family-centered clinical case management services across residential, family-based, and community settings, with a primary focus on Expanded Crisis Support programs. The role partners with youth, families, and caregivers to assess needs, coordinate services, and support stability, safety, and progress toward treatment and life goals. Working collaboratively with interdisciplinary teams, residential staff, Resource Caregivers, and external service providers, the position ensures continuity of care and effective service coordination. Responsibilities include maintaining an assigned caseload, participating in treatment team and IEP meetings, advocating for youth across service systems, and completing timely, accurate documentation in compliance with program and funding requirements. This position is under the direct supervision of the Senior Program Director with additional clinical supervision provided by the assigned external Clinical Program Manager.
Essential Duties:
Behave in a manner that supports the agency's mission and follows personnel policies and procedures.
Provide individual and family-centered clinical case management services using a strengths-based, trauma-informed approach. Partner with youth and families to assess needs, coordinate services, and support progress toward treatment, stability, and life goals.
Deliver clinical case management services across assigned partnering programs, with priority focus on Expanded Crisis Support (ECS). Provide relief or supplemental support within Intensive In-Home and Intensive Independent Living Skills programs as assigned, adapting services to meet the unique needs of youth across residential, family, and community-based environments, including safely transporting youth to and from therapeutic sessions, appointments, and other treatment-related or program activities as required.
Maintain a minimum billable caseload as determined by the Supervisor while ensuring timely, ethical, and high-quality service delivery. Balance productivity expectations with individualized care and adherence to program and funding requirements.
Work collaboratively with residential staff and Resource Caregivers to ensure clinical needs are met within the youth's living environment. Provide guidance, consultation, and coordination to support consistent implementation of care plans and crisis response strategies.
Collaborate with internal and external service providers in a team-based treatment setting and coordinate care with external agencies to ensure continuity of services. Actively participate in cross-system communication to support comprehensive, well-coordinated care.
Advocate for youth across behavioral health, education, child welfare, and related service systems. Attend and participate in treatment team meetings, IEP meetings, and other collaborative forums to support youth access to services and achievement of treatment and educational goals.
Complete and maintain accurate, timely, and comprehensive documentation in accordance with program, regulatory, and funding requirements. Documentation includes assessments, treatment plans, supervision notes, progress reports, discharge summaries, and other required records, submitted by established deadlines.
Minimum Qualifications:
Education / Training:
Bachelor's degree in social work, psychology, or a related human services field from an accredited institution.
Mental Health Professional (MHP) or Paraprofessional Level 2 as required by the Child and Adolescent Mental Health (CAMHD) Performance Standards.
Experience:
Minimum of two (2) years' experience providing clinical and/or therapeutic services to children and families, required.
Experience working children and families who experienced child abuse and neglect or experienced trauma and loss, highly desired.
Other:
Be at least 18 years of age.
Possesses and maintains a valid driver's license
Has daily access to an automobile that meets Hawaii Department of Transportation standards, children under the age of 13 yrs must sit in the backseat while in an operating vehicle and children until the age of at a minimum of 9 yrs old) must be secured in a child safety care seat/booster seat.
Can obtain and maintain current and valid proof of Hawaii State Safety Check.
Can obtain and maintain proof of no-fault insurance.
Can obtain an acceptable pre-hire driver's abstract and annually thereafter.
Can obtain an acceptable Fingerprint (FBI) Background Check upon initial hire and annually thereafter.
Can obtain a State Criminal History check upon initial hire and annually thereafter.
Can obtain an acceptable CWS child abuse registry check upon initial hire and annually thereafter.
Can obtain an acceptable National Sex Offender child abuse registry check upon initial hire and annually thereafter.
Can obtain and maintain current First Aid / CPR certification.
Must have a cell phone for use in emergencies at off-site locations.
