A bachelor's degree in human services or a related field.
Registered as a QMHP with the Virginia Board of Counseling.
One (1) year of position-related experience providing community-based mental health and case management services to the SMI and SUD populations.
Valid Driver's License with a point balance of -6 or higher.
- Knowledge of services and systems available in the community include primary health care, support services, eligibility criteria and intake processes and generic community resources.
- Comprehensive understanding of the nature of serious mental illness, intellectual disability, substance use disorders, or co-occurring disorders depending on the individuals served, including clinical and developmental issues.
- Knowledge and understanding of different types of assessments, including functional assessment, and their uses in service planning.
- Competency in treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.
- Knowledge of types of mental health, developmental, and substance abuse programs are available in the locality.
- Understanding of the service planning process and major components of a service plan.
- Excellent oral and written communications.
- Knowledge and understanding of the use of medications in the care or treatment of the population served.
- Knowledge and understanding of all applicable federal and state laws and regulations and local ordinances.
- Ability to identify and document an individual's need for resources, services, and other supports.
- Ability to use information from assessments, evaluations, observations, and interviews to develop service plans.
- Ability to identify and document how resources, services, and natural support such as family can be utilized to promote achievement of an individual's personal habilitative or rehabilitative and life goals.
- Ability to successfully coordinate the provision of services by diverse public and private providers.
- Ability to work as team members, maintaining effective inter- and intra-agency working relationships.
- Ability to work independently performing position duties under general supervision.
- Ability to engage in and sustain ongoing relationships with individuals receiving services.

We are offering a great opportunity for a full time Case Manager – PSH (Permanent Supporting Housing) with our Adult Service department.
Responsibilities
Perform case management duties with Seriously Mentally Ill adults actively participating in the PSH program. Assist individuals in accessing needed services that are responsive to the person's individual needs. Coordinate and monitor the individual's services with other agencies and supports and assists consumers in gaining access to needed medical, social, educational, and other services. Enhance community integration, makes collateral contacts, participates in discharge planning, and advocates for individuals in response to their changing needs, primarily emphasizing assistance with the consumer's housing needs. Work closely with the housing specialist and supervisor to ensure all consumer needs are met within the scope of the program rules and regulations.

May be required to travel within the Greater Reach CSB catchment area.
Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection. Our Agency maintains a drug-free workplace.
Annual Salary Range
Grade S-C7
$52,576.00 – $59,148.00
