Qualifications


What we require from you:

• Active Dental License in the state you intend to practice
• Oral Surgery Certificate from an accredited U.S. Dental University
• General Anesthesia Permit
• Valid CPR Card
• National Provider Identifier (NPI) Number
• Drug Enforcement Administration (DEA) Certificate
