Description/Job SummaryCrossroads is hiring for a full time Blended Case Manager
Snapshot of the Case Manager Position:

• Community-Based
• Hourly Rate of $19.32 per hour (including an on-call stipend of $90)
• $1,500 sign-on bonus after completion of 180 days of employment
• Monday- Friday 8am-4pm with rotating on-call responsibility
• Qualified Candidates will have a Bachelor's Degree in Human Services related field
• Company Fleet Available

This is a community-based position, working directly with adults diagnosed with a mental health diagnosis in the community to provide support in crisis plannning, accessing services, reaching goals and linking/referring to resources to community resources to maintain independence, stability and self-sufficiency. All employees are provided a cell phone and laptop along with access to company vehicle during working hours.

Responsibilities/Duties

• Assessments
• Goal Planning
• Linkage and Referrals to community resources
• Gaining access to services
• Monitoring of services delivery
• Crisis Planning and problem resolution
• Informal support network building
• Following all operational procedures and regulations
• Assisting clients in attending community appointments
• Moving clients into their residences
  **On-call after hours/weekends on a weekly rotation basis (on-call stipend included)**
  
  

Required ExperienceBi-lingual and previous experience with case management and/or Mental Health/Drug and Alcohol desired.
Required Education Bachelor's degree in a human service field.
Required Qualifications

• Ability to pass a Background Check and Child Abuse Clearances
• The ideal candidate manages multiple priorities, is flexible, and has excellent communication skills.
• Must be able to bend, stand, and sit, for long periods of time and lift up to 25lbs.
• Must possess a valid driver's license, car insurance and a registered/inspected vehicle and live with a 30 minute radius (approximate) due to on call responsibilities.
