    *If so, Baltimore Medical System is your dream come true**The Registered Nurse/Care Manager* assists clinicians in the development of the plan of care for patients. RNs are responsible for the care management and coordination of a patient panel and act as an advocate, educators, and resource persons for the respective population. RN manages the logistical, financial, emotional, educational, and social barriers to care and compliance using the health-team model of care. Will plan, assess, and evaluate the patient's clinical status ensuring treatment plan and medication administration compliance through preplanning activities, care rounds, and patient engagement. #BMSSJ
    *Schedule/Hours*Monday thru Friday: 8:00 AM - 5:00 PM (one late day, depending on center location)
    Occasional Saturday: 9:00 AM - 1:00 PM

    *Essential Functions:
    • Assess care management patients initially, periodically, and at critical junctures for comprehension of their clinical condition, understanding of important steps in the care plan, confidence in self-management, motivation, and ability to persevere in the face of emotional and other obstacles.
    • Collects health status data relevant to the patient's present problem in a continuous, systematic, and complete manner. Completes physical assessment and correlates with patients' health history to prepare the patients for examination or treatment as appropriate.
    • Assures for the provision of teaching/ learning opportunities for the patient. Conducts patient/ family/ significant other educational assessment and develops teaching plan including education material design.
    • Develops an interactive care plan with the patient and the care team.
    • Conducts panel reviews using registries and data dashboards to coordinate and plan care for patients with chronic illnesses.
    • 2+ years of experience as an RN in a pediatric, family practice, or community health setting

    Graduate of an accredited school of professional nursing by date of hire


    Preferred:
    BSN** Registered Nurse in the State of Maryland (or compact state)

    • Unexpired CPR certification from the American Heart Association or American Red Cross
    • Spanish speakers preferred
