- Conduct clinical interventions, direct care visits, and necessary re-evaluations.
- Provide continuous evaluation and treatment of chronic, co-morbid illnesses.
- Evaluate and treat residents for any changes in condition.
- Review medications, orders, and diagnostic tests, and respond appropriately to clinical abnormalities.
- Coordinate care with collaborating physicians for comprehensive medical management.
- Perform Annual Wellness Visits and History and Physicals as required.
- Assist with facility-wide illnesses in collaboration with other healthcare professionals.
- Communicate and coordinate care with other disciplines such as Therapists, Specialists, and Social Workers.
- Maintain family communication and attend conferences whenever possible.
- Complete any other assigned duties.
- Valid nurse practitioner or physician assistant license in the state of operations & current board certification.
- Masters Degree in Nursing or Physician Assistant Studies.
- Preferably, possess strong leadership, interpersonal, and problem-solving skills.
- Excellent written and oral communication skills are a must.
- Must demonstrate punctuality, organizational skills, and teamwork ability.
