    As a Primary Care Nurse Practitioner, you will play a pivotal role in delivering comprehensive healthcare services, promoting wellness, and managing acute and chronic health conditions for patients of all ages.


    Responsibilities:

    Health Assessments:

    Conduct comprehensive health assessments, including physical examinations, health histories, and review of systems, to evaluate patients' overall health status and identify health concerns or risk factors.


    • Diagnosis and Treatment: Utilize advanced diagnostic skills and evidence-based guidelines to diagnose and manage acute and chronic health conditions, such as hypertension, diabetes, respiratory infections, and musculoskeletal disorders.
    • Prescriptive Authority: Prescribe medications, treatments, and therapeutic interventions within the scope of practice and in accordance with state regulations and practice guidelines.
    • Health Promotion and

    Disease Prevention:

    Provide health education and counseling to patients and families on preventive care, health maintenance, nutrition, exercise, and lifestyle modifications to promote overall wellness and reduce health risks.


    • Chronic Disease Management: Develop and implement individualized care plans for patients with chronic conditions, monitoring disease progression, managing symptoms, and coordinating multidisciplinary care to optimize outcomes and quality of life.
    • Acute Care Management: Evaluate and manage acute illnesses and injuries, including minor injuries, infections, and exacerbations of chronic conditions, providing timely and appropriate interventions and referrals as needed.
    • Collaborative Care: Collaborate with physicians, specialists, nurses, and other healthcare professionals to coordinate comprehensive care and services for patients, ensuring continuity of care, effective communication, and interdisciplinary collaboration.
    • Documentation and

    Record Keeping:

    Maintain accurate and up-to-date medical records, documentation, and billing codes in electronic health record systems, adhering to privacy and confidentiality regulations.


    • Quality Improvement: Participate in quality improvement initiatives, performance monitoring, and outcome measurement activities to enhance the delivery of primary care services and promote patient safety and satisfaction.
    *Requirements:
    • Master of Science in Nursing (MSN) degree from an accredited nurse practitioner program.
    • Current licensure as a Nurse Practitioner (NP) and Registered Nurse (RN) in the state of practice.
    • Certification as a Nurse Practitioner in the specialty area of primary care (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner).
    • At least 3 years of clinical experience as a Nurse Practitioner in a primary care setting, required.
    • Strong clinical assessment, diagnostic reasoning, and treatment planning skills.
    • Excellent communication, interpersonal, and patient engagement skills.
    • Commitment to providing culturally competent, patient-centered care in a diverse community.
    • Proficiency in electronic health record systems and medical documentation practices.
    _Please note- The annual base salary range is $110k to $145k.

    Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
