    Job Description:

    We are seeking a skilled Case Manager to provide on-site case management services, support groups, and socialization opportunities to individuals who are pregnant or have recently been pregnant, and their families.

    The ideal candidate will be responsible for linking families with basic needs, physical health appointments, mental health appointments, housing issues, and education and employment opportunities. This includes arranging, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way.

    As part of our multidisciplinary team, you will work as a single point of contact to help clients access the resources they need to recover. You will also be responsible for Oklahoma Department of Mental Health and Substance Abuse Services and Medicaid billing.

    Requirements:

    • Bachelor's Degree in a social sciences field (consideration given for degrees in Criminal Justice or Education)
    • Case Management Certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire
    • Strong commitment to the right and ability of each person served to live in normal community residences
    • Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues and respect for client rights and personal preferences in treatment
    • Valid Driver License and satisfactory driving record; use of personal automobile to travel to locations other than primary office
