    Description
    Summary: The Certified Nurse Practitioner (CNP) is a registered nurse with special knowledge and skills in health promotion and maintenance, disease prevention, physical and psychosocial assessment, and management of health and illness in family practice specialty. The CNP is a primary health care provider who functions in collaborative relationship with qualified physicians within the framework of jointly approved protocols. The CNP maintains responsibility and accountability for actions within this advanced-practice role including prescribing and providing care and making referrals as appropriate.

    Duties and Responsibilities: The authority and duties of the Certified Nurse Practitioner shall include but not be limited to:
    • Obtains a complete health and psychosocial assessment that includes health history and physical examination; records findings in a systematic, accurate, and succinct form.
    • Implements treatment regimens for acute and chronically ill patients according to established protocols.
    • Institutes known procedures for disease prevention.
    • Recalls patients and provides information to the patient and his/her family about diagnosis or plan of therapy following physician-nurse appraisal.
    • Orders, performs, and interprets routine laboratory tests.
    • Initiates requests for and reviews X-ray reports for normalcy.
    • Accepts and initiates referrals for teaching and follow-up care to other members of the health care team, as needed, and to community resources, as appropriate.
    • Provides assessment, education, and management of individuals/families in family planning.
    • Develops and implements plans for health promotion and health maintenance, including plans for disease prevention and provision of health education and counseling.
    • Provides prenatal care for women with uncomplicated pregnancies, giving attention to both maternal and fetal health, according to established protocols.
    • Provides postpartum care, including attention to parent and infant relationships, for women who have had uncomplicated pregnancies.
    • Co-manages care for patients with selected health complications, in collaboration with the physician.
    • Participates in promoting VHcS programs through the performance of off-site health screening.
    • Attends in-services and training, participates in team conferences, quality councils and peer review of medical record audits, as necessary.
    • Carries out many procedures exercising independent judgment.
    • Develops and periodically reviews protocols guiding collaborative practice.
    • Maintains and enhances a current level of knowledge relative to professional practice, as well as continuing education requirements necessary for licensure and certification.
    • Follows all policies, procedures, and standards of VHcS.
    • Nurse Practitioner's annual encounter rate at the Center must exceed the minimum productivity standards of 663 billable patient visits per calendar quarter (2,650 per year) for medical services.
    • Nurse Practitioner may travel between all VHcS locations to perform services if needed.
    • Performs other duties as assigned.
    Evaluation: The evaluation of work performance, including: specific duties, responsibilities, demeanor, and work ethic, will be ongoing and implemented by the Chief Medical Officer per center policies.

    Compliance: Adhere to company policies and procedures and all Federal, State and Local medical service requirements. To include HIPAA Privacy & Security.

    Confidentiality (HIPAA): Every employee is required to sign a Pledge of Confidentiality. Failure to comply with our Pledge of Confidentiality can or will lead to immediate termination of employment. Confidentiality applies to a patient's Personal Health Information (PHI) and includes written, oral and electronic communications. Each employee has an obligation to maintain confidentiality after his or her employment/association with this facility ends.

    Security (HIPAA): Every employee is required to protect his or her computer log-on and password. You must secure your work area prior to leaving it unattended for an extended period. You are required to log-off of your computer and secure your work area prior to leaving at the end of the day. If you have a responsibility for closing or locking files, securing equipment, or locking the building and you do not comply with these requirements, you will be subject to reprimand and/or termination based upon the infraction. We are required by law to protect patient privacy, identifiable patient information, and all properties containing such information.

    Environmental Working Conditions: Works in normal patient care environment where there are minimal physical discomforts due to temperature, noise, dust, etc. Frequently exposed to moderately adverse working conditions in performing certain distasteful patient care activities. Some exposure to communicable disease with little likelihood of harm, if established health precautions are followed. Space is available and accessible for deskwork and private conference with clients and other health care providers.

    Requirements

    Minimum Qualifications:
    • Master of Science Degree in Nursing
    • Successful completion of an advanced nurse practitioner education program, accepted by the American Nurses' Association, the Nurses' Association of the American College of Obstetrics and Gynecology, or the National Association of Pediatric Nurse Associate and Practitioners.
    • Holds a certificate as a nurse practitioner in a specialty area of nursing one of the above certifying agents
    • Holds a current license to practice as a registered nurse in the State of Georgia. Has a minimum of two years of related clinical nursing experience.
    • Professional knowledge of nursing theory and practice. Extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology and management of acute and chronic patient/family problems.
    • High level of analytic ability to concentrate and pay attention to detail.
    • Substantial interpersonal skills necessary to instruct patients and their families and to collaborate with health team members and to effectively communicate verbally and in writing.
    • Ability to read, comprehend and transmit detailed instruction, in order to plan and perform job duties.
    • Ability to concentrate and pay close attention to details; also, ability to organize, prioritize, and work independently, as well as schedule and produce in a timely manner.
    • High degree of integrity to keep confidential all information related to patient health care.
    • Physical ability to work and stand frequently for extended periods.
    Physical Requirements: Varied activities include standing, walking, reaching, bending and lifting. Employee must possess full range of body motion including the ability to assist or lift patients when necessary; occasional weight bearing to exceed 50 pounds, use of portable stretchers/wheelchairs encouraged. Employee must possess hand-eye coordination with normal range of vision and hearing abilities. This position frequently requires working under stressful conditions and frequently requires irregular work hours.
