Job summary
The Outpatient Case Management Nurse position will coordinate the interdisciplinary plan of care for the non-hospitalized population (multi-visit patients, TCM, addressing SDOH) for SRHS service area. Acts as a liaison between the patient/family and healthcare personnel to ensure all necessary care is provided and promotes quality, cost effective care and patient outcomes.
