CASE MANAGER / CASE WORKER– BEHAVIORAL HEALTHCARE

Full Time - Mt. Carmel Guild Behavioral Health System

Positions available in the following locations: Jersey City, Union City, Newark and Cranford

Key responsibilities include, but are not limited to:

• Provide case management and support to consumers in Partial Care and/or Outpatient programs
• Facilitate groups in accordance with professional standards and requirements of agency licensing/accrediting bodies
• Work as part of a multidisciplinary team that provides clinical care and consultation services
• Be responsible for delivering a variety of behavioral health services which include; case manager and treatment and discharge planning
• Document services rendered in a manner and in accordance with the company policies and procedures
• Maintain contact with family and/or providers in the community on behalf of the consumer

To qualify, you will need:

• Bachelor's Degree in a Human Services Field
