- Provides appropriate and optimal service to all patients which may include but not limited to:
- Records patient's medical history
- Performs physical examinations
- Obtains psychosocial history relevant to patient care
- Assesses and creates patient-specific treatment plans
- Provides appropriate treatment interventions as indicated
- Completes follow-up evaluations of ongoing cases
- Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues
- Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs
- Records physical findings, and formulates plan and prognosis, based on patient's condition
- Discusses case with physician and other health professionals to prepare comprehensive patient care plans which may include consultation, collaboration and referral
- Submits health care plan and goals of individual patients for periodic review and evaluation by physician
- Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures
- Where state law permits, may engage in independent practice
- Performs other duties as assigned/necessary
- Current Nurse Practitioner Certification and license in the state of practice required
- Certification as a Nurse Practitioner by a nationally accredited nursing organization (e.g. American Academy of Nurse Practitioners - AANP, American Nurses Credentialing Center - ANCC) is preferred
- Specialty Certification as per client request and or state regulations if applicable
- One year experience as an NP preferred
- Maintain collaboration agreements, per state requirements
- Complies with all relevant professional standards of practice
- Participation and completion of Amergis' Competency program when applicable
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state and local requirements
- Successful completion of new hire training as applicable to job site
- Understand patient confidentiality and HIPAA requirements
- Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
- Computer proficiency required
- Must be at least 18 years of age
