    Services will be provided in accordance with current clinical practice guidelines and accepted professional standards of practice. Care and services are provided according the established plan of care and within the organizations policies and procedures. Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation.

    Manages, directs, coordinates and collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.

    Uses knowledge of patient's age and cultural diversity in the provision of patient care.

    Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care.

    Teamwork, flexibility and professionalism, while important in all functions of the Health System, are central to the success of the Home Health Team, and are expected in this role.

    Travel is required based on the needs of patient caseload and needs of the agency.

    Quality patient care, coordination of services, collaboration, communication and management of a select group of patients through an entire home care episode.

    Develops and initiates the plan of care and revises as necessary in partnership with the patient and/or caregiver. On an ongoing basis will facilitate the patients and caregivers ability to participate in decisions around their care. Provision of services and/or treatments requiring substantial and specialized nursing skill. Educating and counseling the patient, caregiver and family in meeting care related needs. Initiating appropriate preventive and rehabilitative nursing procedures. Documenting clinical/progress notes.


    Coordination of services to include:
    referrals to other services, informing the physician and other staff members of changes in the patients condition. Documents evidence of care coordination. Evaluating outcomes of care. Supervising LPNs' and home health aides providing services in the home or via conversation with patient/caregiver per regulatory requirements. Participates in in-service programs and quality improvement activities. Planning of discharge from service. Graduate of an accredited school of professional nursing.
