Job Summary
We are seeking an RN Case Manager to join our team at North Shore at Home Hospice. As a Case Manager, you will be responsible for planning, organizing, developing, and directing the nursing care of hospice patients.
Responsibilities
- Assisting the admission nurse in the coordination of timely admissions
- Providing hands-on clinical nursing
- Care Includes physical psychological spiritual financial comfort for patient families based on hospice philosophy of care Participate Interdisciplinary Team meetings update Plan Care assigned patients identified problems needs Ensure patient delivered appropriate level care With coordination PCP medical director provide management medication profile prevent polypharmacy optimal symptom manage Manage medication profile ensure hospice providing medications related disease process symptom management Ensure all services provided accordance patient plan involve active participation patient family Primary Care Physician Interdisciplinary Team Provide team members with information coordinate services include communicating making assignments visiting nurses home health aide homemaker chaplain social worker volunteer ancillary services ordering medications Durable Medical Equipment supplies Provide consultation supervision CNAs LPNs involved care patients Participate performance evaluation clinical members involved caseload Assist orientation clinical members participating caseload Ensure cost effective practices self others Assist QAPI activities Work team member uphold Hospice Code Ethics Utilize medication formulary possible symptoms Patient triage scheduling symptom management calls IDT Reports Clinical Director as condition changes or change condition require change level care Work Environment Physical Requirements responsibilities position involve significant activities standing lifting bending stooping pushing pulling twisting Employee must licensed driver automobile insured organization requirements Experience may weather issues traveling rain snow adapt work environment meet patient needs Required Education Graduated accredited program currently licensed good standing Wisconsin State Board Nursing Preferred Education One two years experience acute setting one year experience home health hospice comparable experience desire demonstrate aptitude hospice philosophy comfort Must ability work cooperatively disciplines including social workers chaplains CNAs volunteers Benefits Comprehensive benefit package includes Health Insurance full-time employees starting month following days employment Dental Vision Insurance full part-time staff 401K Program full part-time staff paid holidays floating holiday full-time staff Company Life Insurance Staff Voluntary Life Disability Insurance Full-Time Staff Paid Time Off Mission establish culture reinforces values necessary premier health services provider employer communities serve We understand choosing where want go career big decision like opportunity show Right Choice you
